Are You Aware of the Importance of Advance Directives?

We offer a new kind of volunteer training that enables you to help your fellow community members to complete their advance directives. Our Advance Care Planning (ACP) volunteers tell us this free service to our community feels important and deeply gratifying.

We will train you in the basics of advance care planning.  You will empower people to clarify their end of life wishes, communicate them to loved ones and healthcare providers, put their plans in writing, and register their wishes with a Vt registry.
